The lower half of this fluke formed around the caudal spine, while the upper half was made of soft tissue. This setup suggests the animal was tail-propelled, using its limbs mainly for steering.
The most striking feature of the Ophthalmosaurus skull was the size of its eyes. The lateral area of the cranium was almost entirely occupied by these massive organs. The eyes were so large that the diameter of the outer margin of the bony sclerotic ring was significant. The sclerotic aperture, or the opening for the eye, was also quite large. This specialized anatomy allowed the animal to see in its environment. The skull also featured a long, slender rostrum, which is a snout, filled with many small but robust teeth. 
Scientific understanding of this genus has changed significantly since its discovery. Harry Seeley first described Ophthalmosaurus in 1874, focusing on the morphology of the clavicular bones. Over the following years, many different genera were classified as Ophthalmosaurus. In a 2000 study, Maisch and Matzke listed several names, such as Undorosaurus and Mollesaurus, as junior synonyms. A junior synonym is a name that is no longer used because the animal is now part of a different group. However, more recent studies have contested these conclusions. For example, the genus Undorosaurus is now accepted by most authors. Other species, like the Mexican Jabalisaurus, were once referred to Ophthalmosaurus before being recognized as a distinct genus in 2021.
Taxonomy, the science of naming and classifying organisms, remains a complex field for this genus. In 1880, Marsh named a species Baptanodon, but it was later considered an American species of Ophthalmosaurus. Recent analysis suggests the species might actually be closer to other ophthalmosaurines. Similarly, the species Ophthalmosaurus chrisorum, found in Canada, was moved to the genus Arthropterygius in 2010. Classification within the Ophthalmosauridae family has also shifted. While it was once thought to be most closely related to Aegirosaurus, recent cladistic analyses place it in a clade, or group, with Acamptonectes and Mollesaurus.
The teeth of Ophthalmosaurus provide clues about its diet and lifestyle. The animal had relatively small teeth with robust crowns that showed signs of slight wear. This tooth morphology is considered intermediary. It was not as large as the teeth of Platypterygius, which hunted large prey like birds and turtles. It also lacked the minute teeth of Baptanodon, which was a soft prey specialist. Because of this middle-ground dental structure, researchers like Fischer et al. conclude that Ophthalmosaurus icenicus was likely a generalist predator. This means it could eat a variety of different smaller prey items.
There is also much debate regarding the diving capabilities of Ophthalmosaurus. Some studies suggest the animal could dive for approximately 20 minutes. If it maintained a cruising speed of roughly 1 meter per second, it could potentially reach depths of 400 meters or more. Such depths would place the animal in the mesopelagic zone, a layer of the ocean that receives very little light. However, other researchers have studied the environment of the Peterborough member of the Oxford Clay. Their findings suggest that Ophthalmosaurus may have inhabited shallower waters instead. These studies indicate the animal might have stayed within a distance of 200 meters from the shore.
