It involves the diagnosis, treatment, and surgical management of various eye diseases and disorders. An ophthalmologist is a physician trained in both medical and surgical eye care. After earning a medical degree, these doctors must complete postgraduate residency training. In the United States, this residency lasts four years. Some specialists also complete a fellowship to focus on a specific aspect of eye pathology. This field is essential because the eye is a fragile organ requiring expert care.

To diagnose issues, ophthalmologists use many sophisticated tools and methods. They perform visual acuity assessments to check how clearly a person sees. Ocular tonometry is used to measure intraocular pressure, which is the pressure inside the eye. Doctors also use a slit lamp examination to look closely at eye structures.

One advanced tool is optical coherence tomography, or OCT. This technological platform allows physicians to assess ocular structures and stage pathological processes. OCT scans are vital for managing conditions like glaucoma and diabetic retinopathy. They can also use fluorescein angiography to visualize the vascular networks in the retina and choroid.

Ophthalmology includes several distinct subspecialties that focus on specific parts of the eye. Some doctors focus on anterior segment surgery, which involves the cornea and the ocular surface. Others specialize in neuro-ophthalmology or ocular oncology, which deals with eye tumors. There is also pediatric ophthalmology, which focuses on eye issues in children, such as strabismus.

Some specialists practice medical retina, treating retinal problems through non-surgical means. Others perform vitreo-retinal surgery to manage diseases in the posterior segment of the eye. This variety of training ensures that every part of the eye can receive expert care.

The history of eye science stretches back thousands of years. The Ebers Papyrus from ancient Egypt, dating to 1550 BC, contains sections on eye diseases. In ancient India, the Sushruta Samhita described 76 different ocular diseases. This text also included 51 surgical methods and various instruments.

During the Greek period, physicians like Aristotle used animal dissections to study the eye. Aristotle discovered that the eye had three layers rather than two. Later, the physician Rufus of Ephesus recognized the conjunctiva as a fourth epithelial layer. He also correctly identified a two-chambered eye structure.

As time passed, medical understanding became much more precise. The physician Galen corrected several errors regarding the cornea, lens, and optic nerve. In the 18th century, scientists used microscopes and specialized preparations to study the eye in detail.

A major milestone occurred on September 18, 1750, when Jacques Daviel performed the first documented planned primary cataract extraction. In the United Kingdom, the first dedicated ophthalmic hospital opened in London in 1805. This institution is known today as Moorfields Eye Hospital. It became a global center for ophthalmic research and clinical development.

Ophthalmologists treat a wide range of serious conditions. These include cataracts, which cloud the lens, and glaucoma, which affects eye pressure. They also manage retinal detachment, macular degeneration, and diabetic retinopathy.

Other treated conditions include uveitis, eye injuries, and orbital fractures. Even refractive errors, which affect how light enters the eye, are managed in this field. Because these diseases can lead to vision loss, the work of ophthalmologists is vital for maintaining sight. Pharmaceutical companies like Regeneron and Bausch Health develop important products to treat these ailments.

Modern ophthalmology is deeply connected to technology and broader medical research. Many ophthalmologists participate in academic research to discover new ways to treat eye diseases. The field relies on physics and engineering for tools like electroretinography (ERG). This technique measures the electrical responses of cells in the retina, such as rods and cones.

Other technologies include corneal topography, which maps the curvature of the cornea. This intersection of medicine and technology continues to drive innovation in how we protect and restore human vision.
