The canal starts at the Dortmund-Ems Canal near Hörstel. From there, it runs north past the Teutoburg Forest. It goes past the city of Hanover. Eventually, it meets the Elbe River near Magdeburg. Near Magdeburg, it connects to the Elbe-Havel Canal. This creates a long route that leads all the way to Berlin.

Building this long canal took many years. Workers started the project in 1906 at Bergeshövede. The first part opened in February 1915. At that time, people called it the Ems-Weser-Kanal. To navigate the landscape, the canal uses specialized engineering structures. At Minden, the canal crosses the Weser River using two different aqueducts. An aqueduct is a bridge designed to carry water across another body of water. The first Minden aqueduct was completed in 1914. A second one was finished in 1998. The canal also crosses the Elbe River using an aqueduct near Magdeburg. These structures allow boats to maintain their path without being blocked by the rivers below.

Building this massive network was a long process that spanned decades. Construction began in 1906 at Bergeshövede on the Dortmund-Ems Canal. The first completed section opened in February 1915. History significantly impacted the canal's operation and purpose. After the Second World War, Germany was partitioned into two separate states. The Mittelland Canal was also split between West Germany and East Germany. The border was located just east of Wolfsburg. To help ships reach Hamburg without entering East Germany, the Elbe Lateral Canal opened in 1977. This canal provided a way to avoid the Elbe River's limited navigability during summer. Following the revolutions of 1989, the canal's importance grew as Germany reunified.

Modern engineering solved old problems facing the waterway. For a long time, a planned bridge over the Elbe was not built due to the Second World War. This bridge was necessary to avoid low water levels during the summer months. After the fall of the Iron Curtain, the project to bridge the Elbe was restarted. The Magdeburg Water Bridge finally opened in 2003. This bridge provides a direct link to the Elbe-Havel Canal. This makes the connection between the west and the east much more efficient.

The canal also connects to many other local waterways through side canals. These connections exist at several locations including Ibbenbüren and Osnabrück. Other branch canals connect to Minden, Hanover-Linden, and Hanover-Misburg. There are also connections at Hildesheim and Salzgitter. West of Wolfsburg, the Elbe Lateral Canal branches off. This provides a route to Hamburg and the Baltic Sea via the Elbe-Lübeck Canal. These branches turn the main canal into a complex web of transport.

The canal has played a vital role in moving essential resources. For example, the Mehrum Power Station used the canal to receive supplies. The station received bituminous coal via the waterway for many years. The station remained in operation until it closed on April 1, 2024. While some projects are not always profitable, the canal remains a central part of the system. In 2013, a plan to connect the canal to the Twentekanaal in the Netherlands was considered. However, that specific project was deemed unprofitable.
