President Vladimir Putin chose him for this job on March 1, 2004. Many people were surprised by this choice because Fradkov was seen as an outsider. He did not belong to any of the big political groups in the Kremlin. Some leaders said he was a professional who was independent of any political clans. His government worked on many big projects for the people of Russia.

His team started important work like the National Project for Health and the National Project for Education. They also worked on making housing more affordable for citizens. In 2004, they began a plan to reorganize government departments. One project involved changing how people received social benefits by using money instead of goods. This change caused some protests in early 2005. Fradkov resigned as Prime Minister on September 12, 2007. After that, he led the Foreign Intelligence Service until 2016.

Fradkov has received several high honors for his work in Russia. He earned the Order of Merit for the Fatherland in both the 1st and 2nd classes. These awards recognized his help with the country's economy. However, his later years included some difficult news from other countries. In 2018, the United States put sanctions on him. The United Kingdom also added sanctions in 2022 due to the Russo-Ukrainian War. Since 2017, he has served as the Director of the Russian Institute for Strategic Studies.
