During this early period, they recorded the demo "Pure Fucking Armageddon." They also released the EP "Deathcrush" in 1987 through Aarseth's label, Posercorpse Music. The first pressing of "Deathcrush" was limited to 1,000 copies and sold out quickly.

In the late 1980s, the band's identity changed with new members. Swedish vocalist Per Yngve Ohlin, known as "Dead," joined the group. He was accompanied by drummer Jan Axel Blomberg, known as "Hellhammer." Dead introduced a specific visual style called "corpse paint." This involved using black and white makeup to look like a corpse. He did not use this makeup to look fashionable. Instead, he wanted to achieve a ghastly, non-human appearance.

His stage presence was highly theatrical and extreme. He often used hunting knives or broken glass during his performances. The band also used animal heads on stakes to enhance the dark atmosphere.

Tragedy struck the band in April 1991. Dead committed suicide in the house where the band lived near Oslo. He left a note explaining that he did not feel human. Following the suicide, guitarist Euronymous used the event to build an "evil" image for the band. He even photographed the scene and made necklaces from fragments of Dead's skull. These actions caused deep tension within the music community. Bassist Necrobutber was shocked by Euronymous's reaction to the death. This period of instability marked a dark chapter in the band's history.

The violence surrounding the band continued into 1993. In August of that year, Varg Vikernes murdered Euronymous. Vikernes was a former bassist for Mayhem who also played in the band Burzum. He traveled 518 km from Bergen to Oslo to commit the act. The murder involved twenty-three stab wounds. Vikernes was later sentenced to 21 years in prison. He was also linked to a series of Norwegian church burnings. These events caused Mayhem to effectively cease to exist for a short time. The band's debut album, "De Mysteriis Dom Sathanas," was delayed because of this murder.

Mayhem eventually reformed in 1995. The surviving members included Necrobutcher, Hellhammer, and vocalist Sven Erik Kristiansen. They brought in new musicians, such as guitarist Rune Eriksen, known as "Blasphemer."
