In 1350, Hayam Wuruk inherited the throne at age 16. He took a new regnal name: Sri Rajasanagara Jayawishnuwardhana. He ruled after his mother, Queen Tribhuwana, had to abdicate. This happened after the death of Queen Dowager Gayatri in 1350. Gayatri had been the primary consort of the first Majapahit king. During his reign, the empire reached its peak of strength. In 1365, Mpu Prapanca wrote the Nagarakretagama to honor him. This manuscript describes the king's travels through East Java. He visited villages, holy shrines, and vassal kingdoms. He also sent ambassadors to China between 1370 and 1381.

His personal life included a very difficult period known as the Bubat incident. In 1357, a marriage was arranged with Princess Dyah Pitaloka Citraresmi. She was the daughter of the King of the Sunda and Galuh kingdoms. The marriage was meant to create a political alliance. However, a skirmish occurred between Majapahit troops and the Sunda royal party. This conflict resulted in the death of the princess and her party. The court officials blamed Gajah Mada for the bloodshed. Following this, Hayam Wuruk married his relative, Paduka Sori.

Hayam Wuruk had a daughter named Princess Kusumawardhani. He arranged for her to marry her cousin, Wikramawardhana. This marriage happened when the princess was only 7 years old. He also had a son with a concubine. This son was the 2nd Prince of Wirabhumi. When Hayam Wuruk died in 1389, the empire entered a time of chaos. There was a contest over who should lead next. This dispute involved Wikramawardhana and Wirabhumi. The conflict eventually ended with the defeat of Wirabhumi in the Regreg war.

After the conflict, Wikramawardhana succeeded him as the King of Majapahit. The legacy of Hayam Wuruk is still studied today. His reign helped spread Hindu culture and Sanskrit traditions throughout the region. This process is often called Indianisation. The cultural influence of his time can be seen in Javanese worldview. Elements of the Hindu epics, the Ramayana and the Mahabharata, became part of the culture. They were shared through the use of wayang kulit, or leather puppets. His era remains a central part of Indonesian history.
