He was known to many people by his nickname, "Manong." Although he was trained as a medical doctor, his true passion was making movies. He first appeared as an actor in the 1934 film Ang Dangal. After acting in eight more films, he began directing his own stories. His first movie as a director was called Bahay-Kubo in 1939. This film starred Fely Vallejo, who later became his wife. He belonged to a large family of filmmakers known as the Ilagan clan.

Making movies involves many different jobs and steps. De Leon started by acting in front of the camera. Later, he took on the role of a director to lead the whole project. He also worked as a producer and sometimes a writer. In the 1960s, he even helped finance some horror films with American money. He often worked with friends, like director Eddie Romero. These collaborations helped him create many different kinds of stories for the screen.

His life was full of big changes and hard moments. During World War II, he worked with Japanese forces to make films. After the war ended, he faced very serious legal trouble. The government thought he had helped the wrong side during the war. However, people found evidence that he had secretly helped the Filipino resistance. This discovery saved him from a very bad fate. He lived to be 67 years old before he passed away in 1981.

De Leon was a master at winning prestigious awards. He won seven FAMAS Awards between 1952 and 1971. The FAMAS is the group that gives awards for Filipino movies. He is the most awarded director in the history of these awards. Every time he won Best Director, his movie also won Best Picture. Some of his famous winning films include Sawa sa Lumang Simboryo and Noli Me Tangere. In 1982, he was named a National Artist of the Philippines for Film.

Today, people still remember his work in many different ways. Some fans love his scary horror movies from the 1960s. These include titles like Terror Is a Man and Brides of Blood. His film The Moises Padilla Story was even sent to the Academy Awards. This was the Philippine entry for Best Foreign Language Film in 1961. His family also stayed involved in the world of acting and movies. His daughter, Liberty Ilagan, was an actress, and his other daughter, Maria, had children who are actors too.
