He also practiced anti-freemasonry. To unify his supporters, he created the FET y de las JONS. This group was also known as the Movimiento Nacional. During World War II, Spain remained officially neutral. However, Franco supported the Axis powers. This support damaged Spain's international reputation during that period.

Franco’s regime was marked by significant human cost and repression. The government used forced labour and concentration camps against political opponents. These actions led to many deaths. Estimates suggest at least 30,000 people died from post-war repression. Some scholars suggest the number could exceed 200,000. When combined with wartime killings, the total death toll is estimated between 100,000 and 350,000.

Despite this repression, Spain also experienced a period of economic change. During the Cold War, Franco adopted technocratic and economically liberal policies. This led to a period of rapid growth called the "Spanish miracle." His regime transitioned from a totalitarian state to an authoritarian one. This new version of the government allowed for limited pluralism. Because of his anti-communist stance, he gained support from the West. The United States, in particular, became a supporter.

In his later years, Franco's health began to decline. He struggled with Parkinson's disease during the 1960s. In 1973, he resigned as prime minister due to his age and illness. However, he kept his roles as head of state and commander-in-chief. He died in 1975 at the age of 82.
