Born on February 21, 1747, in the Royal Audiencia of Quito, he was a medical pioneer, a lawyer, and a journalist. He is remembered as a polemicist who helped inspire the separatist movement in his region. Espejo was also a key hygienist who studied how diseases spread. His work helped introduce the ideas of the Age of Enlightenment to the people of Quito.

Espejo's medical knowledge grew through direct experience. He worked alongside his father at the Hospital de la Misericordia to learn the practice of medicine. He believed that true knowledge required both study and hands-on experience. He graduated from medical school on July 10, 1767. Later, he studied jurisprudence and canon law under Dr. Ramón Yépez from 1780 to 1793. By November 28, 1772, he had official permission to practice medicine in Quito.

As a scientist, Espejo made important observations about public health. In 1785, the town council asked him to write about smallpox. This was the most serious medical problem in the Audiencia. He wrote a treatise titled "Reflexiones acerca de un método para preservar a los pueblos de las viruelas." In this work, he discussed the role of microorganisms in the spread of disease. He also criticized the poor sanitary conditions and the negligence of health officials. His findings were so significant that they were included in a medical book in Madrid in 1786.

Espejo was also a famous satirist who used humor to criticize society. A satirist is someone who uses irony or mockery to point out flaws. He often used a pseudonym, which is a fake name, to protect himself. One name he used was "Moisés Blancardo." Another was "don Javier de Cía, Apéstegui y Perochena" for his work "El nuevo Luciano de Quito." These pseudonyms helped him hide his identity and his mixed racial background. He used these writings to attack corruption, poor education, and the lack of economic management in Quito.

The social structure of the Royal Audiencia of Quito was very strict.

Society was often divided into different groups based on racial origin. This meant that a person's dignity could be hurt by ethnic prejudice. During this time, the economy was struggling due to a lack of roads and the decline of textile factories called obrajes. Education was also in a difficult state. After the expulsion of the Jesuit priests, there were very few learned people left in Quito. Most students used memorization rather than new ideas, which left the city intellectually isolated.

Because of his bold writing, Espejo faced many legal troubles. In 1783, authorities labeled him as "restive and subversive." They tried to move him away from Quito by appointing him as a physician for a scientific expedition. When he tried to flee, he was captured and sent back as a "criminal of serious offense." He was later arrested again in 1787. He was accused of writing a satire against King Charles III. Even while in prison, he continued to fight for his rights by sending petitions to the Court in Madrid.

Espejo's travels eventually led him to Bogotá. There, he met influential thinkers like Antonio Nariño and Francisco Antonio Zea. These meetings helped him develop his ideas regarding liberty and freedom. His life connects the scientific study of health with the political struggle for independence. He remains one of the most important figures in the history of Ecuador.
