It existed from the late 6th century until the early 13th century. The duchy was centered on the city of Spoleto. It controlled many mountains and valleys in central Italy. This included parts of modern Umbria, Abruzzo, Marche, and Lazio. The land was very important because of its location. It sat on a major road called the Via Flaminia. This path connected Rome and Ravenna. By holding this road, the dukes could control movement in the region.

Life in the duchy was organized around kinship groups. These groups were called fare. They were like large extended family clans. These clans formed the basic units for living and organizing. Early settlements were often very simple. People built encampments on hilltops for safety. They used wooden watchtowers and walls made of wood called palisades. As time went on, they moved into more permanent places. They occupied old fortified towns and mountain passes. This helped them keep control over the large territory. They also used old Roman structures for new needs. For example, a cathedral was built inside an old Roman temple.

The duchy began during the Lombard conquest of Italy. This happened in the 570s. A leader named Faroald I helped establish the duchy. Some stories say it started around the year 571. Other stories suggest it began after a battle in 575 or 576. After Faroald I died in 591, Ariulf became the next duke. He grew the duchy by taking land from the Byzantines. He led many trips against Byzantine cities like Ravenna and Rome. Later, a duke named Theodelap ruled until about 650 or 653. He is remembered for building the first cathedral in Spoleto. These early leaders helped shape the identity of the land.

Many dukes had to deal with powerful kings and the Pope. Transamund II was a famous duke who ruled in the 700s. He fought a Byzantine army at the Ponte Salario. He also worked with the Roman Church to defend the Pope. However, the Lombard King Liutprand often tried to take control. Liutprand even marched on Spoleto with a large army. In 757, the nobles elected a new duke named Alboin. This led to conflict with King Desiderius. Desiderius invaded the duchy and caused much damage between 757 and 758. These struggles showed how hard it was to stay independent.

In 776, a Frankish leader named Charlemagne conquered the area. This changed the duchy from Lombard rule to Frankish rule. Charlemagne gave some lands to Pope Adrian I. Later, new families like the Widonids began to rule. One duke named Guy I was a Frank who ruled in 845. He even helped another duke during a siege. By the late 12th century, things began to change again. The duchy was slowly taken over by the Papal States. This means the Pope's government began to rule the land. By the time of Pope Innocent III, the duchy was no longer independent.
