The head of Bothriolepis had a very specific shape. There were two main openings on the top of the head. A keyhole-shaped opening allowed for the eyes and nostrils. Below the head, an opening served as the mouth. This mouth was located on the ventral side, or the bottom. This position suggests the fish rested horizontally on the floor. A special bone partition called a preorbital recess enclosed the nasal capsules.

Bothriolepis had unique ways of moving through its environment. It possessed a slender trunk that was likely covered in soft skin. Instead of pelvic fins, it had a long pair of pectoral fins. These fins were spine-like and had joints at their base. The fish likely used these jointed fins to lift its heavy body. This helped it stay clear of the bottom. It might have also used them to throw sediment over itself.

There is an ongoing debate about whether Bothriolepis had lungs. Some researchers found paired ventral sacs extending from the pharynx. They hypothesized these sacs might have functioned as lungs. This could have allowed the fish to breathe air. It might even suggest they could travel on land. However, other scientists disagree with this idea. They argue there is no direct evidence for lungs in certain fossil samples. They believe the fish may have lived strictly in marine environments.
