Biopsy

life science Maturity 11-13 Next article →

A biopsy is a fundamental medical procedure used to investigate health conditions. Doctors perform this test to determine the presence or extent of a disease. During the process, a specialist extracts a sample of cells or tissues from a patient. This sample is then examined to see if a condition is benign or malignant. Benign means the tissue is not cancerous, while malignant means it is.

Several specialists can perform these procedures. These include surgeons, interventional radiologists, or interventional cardiologists. Once the sample is collected, it must undergo a specific preparation process. The tissue is fixed, dehydrated, and embedded in a material. After that, it is sectioned into thin slices and stained with dyes. Finally, a pathologist examines the specimen under a microscope or analyzes it chemically.

There are different methods for collecting tissue depending on the medical need. An excisional biopsy occurs when a surgeon removes an entire lump or suspicious area. In contrast, an incisional biopsy or core biopsy only samples a portion of the abnormal tissue. Doctors may also use a needle aspiration biopsy. In this method, a needle removes cells or fluid without preserving the histological architecture of the tissue.

When a doctor performs an excisional biopsy for cancer, they check the surgical margins. The margins are the edges of the removed tissue. If the margins are clear or negative, no disease was found at the edges. If the margins are positive, disease was found at the edge. This may mean a wider excision is required to remove the illness completely. A larger specimen removed to eradicate a known lesion is called a resection.

Medical history shows that biopsy techniques have evolved over many centuries. The Arab physician Abulcasis lived from 1013 to 1107. He developed one of the earliest diagnostic biopsies by using a needle to puncture the thyroid. This allowed him to characterize many different types of goiter. The word "biopsy" itself comes from the Greek words for "life" and "a sight." The French dermatologist Ernest Besnier introduced this specific term to the medical community in 1879.

Modern science has introduced a new method called a liquid biopsy. This is not a traditional tissue biopsy but rather a specialized blood test. It looks for circulating tumor cells (CTCs) or cell-free circulating tumor DNA (cfDNA). These methods are non-invasive, meaning they do not require cutting into the body. This makes them useful for monitoring how a cancer treatment is working in real time.

Liquid biopsies offer several advantages over traditional tissue biopsies. Tissue biopsies can be invasive and cannot be used repeatedly on the same spot. Because tumors are genetically diverse, a single tissue sample only provides a snapshot in time. Liquid biopsies can detect changes as the tumor progresses or spreads. A 2014 study of 846 patients showed that tumor DNA was in the blood of over 80 percent of patients with metastatic cancers. This test also found tumor DNA in about 47 percent of patients with localized tumors.

Biopsies can be performed at many different sites throughout the human body. Doctors can take bone biopsies to look for infection or cancer. They can also perform bone marrow biopsies to study blood cell abnormalities like leukemia. In the gastrointestinal tract, doctors use flexible endoscopes to reach the esophagus or stomach. Even the lungs and liver can be sampled using specialized needles or tools.

Beyond cancer, biopsies are used to study many other medical issues. They help diagnose inflammatory conditions like Crohn's disease or ulcerative colitis. Biopsies of the temporal arteries can identify vasculitis. They are also used in organ transplantation to ensure the body is not rejecting the new organ. Even in fertility studies, a testicular biopsy can help find the cause of low sperm quality. These tests are essential tools for understanding the complex systems of the human body.
