Some wounds happen very fast from a sudden injury. These are called acute wounds. Other wounds develop slowly over time because of a disease. These are called chronic wounds. Doctors must look at a wound carefully to help it heal. They check how deep it is and where it is located. They also look at the wound bed to see if it is healthy. A healthy wound bed often looks pink.

When an acute wound happens, the body follows a specific way it works to heal. This process has four main steps. The first step is called hemostasis. This step stops the bleeding and lasts from minutes to hours. Next is the inflammatory phase, which lasts one to three days. The third step is called proliferation. During this stage, the body builds new tissue for a few days or up to a month. The final step is remodeling, which is when a scar forms. This last part can last for 12 months or even up to two years.

There are many different ways a wound can look. An open wound happens when the skin is broken and tissue is exposed. An example is an abrasion, which is a scrape from a rough surface like asphalt. You might also see a puncture wound from a nail or a splinter. Some wounds are caused by sharp objects like knives, which are called incisions. Other wounds are closed, meaning the skin stays in one piece. A hematoma is a closed wound where blood collects under the skin. A bruise is also a type of closed wound caused by an injury.

Sometimes, wounds do not heal on their time. A chronic wound is a wound that is stuck or delayed in its healing steps. Many clinicians say a wound is chronic if it does not progress for over three months. These can happen if a person has a disease like diabetes mellitus. Diabetes can make it hard for the body to heal. Other causes include problems with how blood flows through the body. Pressure ulcers, also called bedsores, can happen if a person stays in one position for too long. In the United States, doctors grade these ulcers from stage 1 to stage 4.

Doctors use many tools to understand a wound. They might use a probe to see how deep a wound really is. They also look for dead tissue, which can look yellow or black. Sometimes they use an X-ray to check for broken bones under a wound.

They also use special systems to see if a wound is sterile. A sterile wound is clean and has no infection. A dirty or infected wound shows signs of a clinical infection. Knowing these details helps healthcare professionals choose the right way to treat the injury.
