The Order's history began around the year 1048 in the Kingdom of Jerusalem. Merchants from the Republic of Amalfi built a church, a convent, and a hospital there. This facility was meant to care for pilgrims of any race or faith. A religious brother named Blessed Gerard led this monastic community. In 1113, Pope Paschal II issued a papal bull to officially approve the foundation. This document placed the hospital under the protection of the Holy See. It also gave the Order the right to elect its own leaders without outside interference. Originally, all members were religious and took vows of poverty, chastity, and obedience.

As the history of the Holy Land changed, the Order moved to several locations. After the Siege of Acre in 1291, the knights settled in Cyprus. In 1310, they moved to the island of Rhodes under Grand Master Fra' Foulques de Villaret. In Rhodes, the Order organized a powerful naval force to sail the eastern Mediterranean. The knights were even grouped into "Langues," or tongues, based on the languages they spoke. These included groups from Provence, Italy, England, and Germany. In 1523, after a six-month siege by the forces of Sultan Suleiman the Magnificent, the knights surrendered Rhodes. However, they did not disappear; they remained a sovereign force with their own coins and diplomacy.

In 1530, the Order received the island of Malta from Emperor Charles V.

This happened under the approval of Pope Clement VII. The knights had to follow specific conditions, such as the Tribute of the Maltese Falcon. In 1565, the Order faced the Great Siege by the Ottomans. Led by Grand Master Fra' Jean de Valette, the knights defended the island for over three months. The capital city, Valletta, was named in his honor. The Order's fleet also played a role in the Battle of Lepanto in 1571. This battle helped destroy Ottoman naval power in the region.

Significant changes occurred during the era of the Reformation and the Napoleonic Wars. The Reformation caused the Order to dissolve in some countries, like England and Sweden. However, "Johanniter orders" survived in places like Germany and the Netherlands. In 1798, Napoleon Bonaparte led a French occupation of Malta.

After fierce fighting, the Grand Master, Ferdinand von Hompesch zu Bolheim, negotiated a surrender. The knights were dispersed, but the Order continued to exist in a smaller form. In Russia, Emperor Paul I provided shelter to many knights. He was even elected as a Grand Master, though this was not ratified under Catholic law.

The modern Order has a complex structure and many members.

It includes about 13,500 Knights, Dames, and Chaplains. Thirty-three of these are professed religious Knights of Justice. While high offices once required proof of noble lineage, new paths now allow members to rise through the ranks. The Order's headquarters are in Rome, where two buildings enjoy extraterritoriality. This means they are treated as special zones under international law. Since 1994, the Order has also served as an observer at the United Nations General Assembly. This role recognizes their long-standing dedication to international humanitarian relations.
