Water is the main thing that shaped these caves. The Weyib River flows from the high Bale Mountains toward the cave. The river hits a wide area of limestone rock. Over time, the water dissolved the limestone to create many underground paths.

The river eventually moved into these new passages. It left behind a dry valley that runs from the Ayiew Maco entrance to the Holuca resurgence. This is where the river comes back out into the sunlight. The river also moves through a place called the Big Rapids. This area is full of huge boulders that the water has worn down.

Many people have explored these dark halls over many years. Arthur Donaldson Smith recorded a visit in 1894. An Italian expedition also visited in 1913. In 1934, Henri Breuil studied the area for archaeology.

Later, in 1967, Eric Robson, Chris Clapham, and Kabir Ahmed surveyed 8 km of the cave. In 1971, Professor Théodore Monod and others found 1 km of new passages. A large British expedition arrived in 1972. This team included many people like Dick Ramsden and Tim Renvoize. They did a full survey and found many more paths. This brought the total survey length to 26 km.

The cave has many different rooms and shapes. One of the most famous parts is the Chamber of Columns. This room has many thick stone pillars packed closely together.

There is also a large area called the Astrodome. This is a big chamber where many bats live together. Some passages, like the Mudwall Passage, show layers of mud. This mud is made by floods that happen every year. The cave also has a very wide path called the Great Hall. This hall is 25 m wide and breaks out into the canyon walls.

You can think of the cave like a giant, natural maze. The river acts like a sculptor, carving paths through the rock. The different passages are like hallways in a very large house. Some hallways are wide and easy to walk through. Other parts, like the Wembley Stadium passage, are tight crawls.

The cave is also shaped by faults in the Earth. A fault called the Wembley Fault helped create a large hole in the ground above. This hole is called a shakehole. It even helped create the Big Rapids by dropping rocks into the river. The cave shows us how water and earth work together over a long time.

While it was once the longest cave in Africa following a 1972 survey, it has since been surpassed by systems in Madagascar and Algeria. Beyond its geological importance, the site holds deep cultural value. It serves as a sacred religious center for both Islam and the local Oromo traditional religion. Because of this unique cultural and natural status, the site was added to the UNESCO tentative list in 2011.

The formation of the cave is a result of complex hydrological processes. The Weyib River, also known as the Gestro River, flows from the high Bale Mountains through a wide outcrop of Anatole limestone. Over time, the river dissolved the limestone to create phreatic passages, which are underground tunnels formed while completely submerged in water. These passages eventually grew large enough to capture the entire flow of the river. As the river moved into these new underground routes, it abandoned its old path. This left behind a dry valley that runs from the Ayiew Maco entrance to the Holuca resurgence, where the river reappears.

The cave's structure is heavily influenced by the Earth's tectonic activity. It is formed along a network of joints, which are natural cracks in the rock. One set of joints runs north to south, while another runs east to west. This creates a zig-zag pattern of passages moving in a southeasterly direction. A major feature is the Wembley Fault, a crack in the Earth's crust. This fault played a significant role in a major collapse that created a large shakehole on the basalt plateau above the cave. This collapse sent underground debris into the river, helping to form the Big Rapids.

Exploration of the system has occurred in several distinct historical waves. The explorer Arthur Donaldson Smith recorded a visit in 1894, followed by an Italian expedition in 1913. In 1934, Henri Breuil conducted archaeological investigations in the area. Modern surveying began in earnest in 1967, when Eric Robson, Chris Clapham, and Kabir Ahmed recorded 8 km of passage. The river also flows through the Big Rapids, a striking feature made of huge, water-worn boulders. Another notable area is the Astrodome, a large chamber that serves as a home for a massive colony of bats. In some sections, like the Mudwall Passage, the environment changes due to seasonal cycles. This passage contains thin laminations of mud, which are layers created by annual flooding.

Navigating the cave requires moving through various types of passages. The Ayiew Maco Series consists of interconnecting paths ranging from 1 m to 10 m in width. Some sections, like the Great Hall, are much larger, reaching a width of 25 m. Other areas are much more difficult to traverse. For example, the Wembley Stadium passage is a tight crawl formed within a fault. The cave also features many entrances, though only four are generally considered useful for entry. These include the Village Entrances near the local shrine and the Tourist Entrance located downstream from the Holuca resurgence.
