He is considered one of the most successful Olympic athletes ever. Over his career, he won twelve Olympic medals. Eight of those medals were gold. This record makes him the most successful Japanese Olympian in terms of gold medals. His career spanned from 1968 to 1976. He showed incredible skill across many different gymnastics events.

Gymnastics involves many specific disciplines that require unique strengths. Katō mastered several of these different events. He excelled in the all-around competition. The all-around measures a gymnast's skill across every discipline. He also won gold medals on the floor and the parallel bars. He earned silver medals on the pommel horse and the horizontal bar. Additionally, he won a bronze medal on the rings. This variety shows his complete mastery of the sport.

Katō's journey began in Niigata Prefecture. He was born there on October 11, 1946. He later attended Tokyo Kyoiku University to further his studies. His Olympic career started at the 1968 Summer Olympics. He did not compete alone during this time. His elder brother, Takeshi, also competed in the games. Together, they won a gold medal in the team competition. Sawao also took gold in the all-around and on the floor.

By the 1972 Olympics, the Japanese men's gymnastics team was a dominant force. They were incredibly successful in individual events. The team won 15 out of 21 available individual medals. Katō was a major part of this success. He won gold medals in the all-around and on the parallel bars. He also took silver medals on both the horizontal bar and the pommel horse. These wins cemented his status as a world-class athlete.

In 1976, Katō aimed for a historic third consecutive all-around gold medal. This would have been an unprecedented achievement. However, he was defeated by Nikolai Andrianov for that specific title. Even without that specific gold, the Japanese team performed brilliantly. The team competition was a very close contest. Japan defeated the Soviet Union by only four tenths of a point. This victory earned Japan its fifth consecutive team title. Katō ended his Olympic career by winning gold again on the parallel bars.

Katō's statistics place him among the greatest athletes in history. He is one of only ten athletes to win eight or more Olympic gold medals.

When looking at male gymnasts, his record is truly remarkable. His eight gold and twelve total medals are best rivaled by a few others. Nikolai Andrianov won seven gold and 15 total medals. Boris Shakhlin won seven gold and 13 total medals. Takashi Ono won five gold and 13 total medals. Katō holds more gold medals than any other Japanese Olympian. He ranks second only to Ono in total medal count.

His legacy continues far beyond the gymnastics arena. In 2001, he was inducted into the International Gymnastics Hall of Fame. This honors his immense contribution to the sport. He also spent time in academia. As of 2010, he was a professor emeritus at the University of Tsukuba. Even space scientists have recognized his name. In 2000, the American LINEAR survey discovered a Jovian asteroid. On May 14, 2021, it was named 43212 Katosawao in his honor.
