History shows that quarantine has evolved from custom into a scientific protocol. In the early 8th century, the Umayyad caliph Al-Walid I practiced the segregation of lepers. In the city of Fez, Morocco, lepers were moved to a cave system outside the city. In 1025, the Persian Muslim polymath Ibn Sina, also known as Avicenna, formalized these ideas. In his medical encyclopedia, The Canon of Medicine, he proposed a systematic and scientific approach to quarantine. He established that isolation was a clinical necessity to prevent the spread of diseases, including tuberculosis.

The term "quarantine" has specific roots in the history of the Black Death. Between 1348 and 1359, the plague killed an estimated 30% of Europe's population. In 1377, the city-state of Ragusa, in modern Croatia, imposed a 30-day isolation period called a trentino. This required newcomers to stay in a restricted place to see if symptoms developed. In 1448, the Venetian Senate increased this waiting period to 40 days. The word comes from the Venetian term quarantena, which means "forty days."

This 40-day period was scientifically effective for the era. The bubonic plague had an estimated 37-day period from the time of infection to death. Therefore, a 40-day quarantine was successful at determining the health of ship crews. Venice led many early efforts, founding the first lazaret, or quarantine station, in 1403. Other cities followed this model, such as Genoa in 1467 and Marseille in 1526. These stations, known as lazarettos, served as dedicated sites for managing infectious arrivals.

Different diseases have triggered various quarantine responses throughout the centuries. In the 15th and 16th centuries, Zurich used quarantine to prevent the spread of syphilis.
