He served as the fourth and seventh prime minister of Malaysia. His leadership spanned two distinct periods: from 1981 to 2003, and again from 2018 to 2020. In total, he served for 24 years, making him the longest-serving prime minister in the nation's history. He is often called the "Father of Modernisation." This title reflects his role in transforming Malaysia's economy and infrastructure. At 100 years old, he is the first Malaysian prime minister to reach this age. He is also the second-oldest living former state leader in the world.

Mahathir was born on 10 July 1925, in Alor Setar, Kedah.

His family background was not aristocratic or religious. His father was a school principal of Malay and Indian descent. His mother was Malay from Kedah. Mahathir grew up in a poor neighborhood without electricity. During the Japanese occupation in World War II, he ran a small business. He sold coffee and snacks like pisang goreng, which are banana fritters, at Pekan Rabu. This early experience with entrepreneurship shaped his later career.

Before entering politics, Mahathir focused on his medical studies.

He attended the King Edward VII College of Medicine in Singapore. He graduated from the University of Malaya in 1953. As a doctor, he worked in several locations, including Langkawi. He witnessed how underdeveloped Langkawi was during his time there. Later, he opened Maha Klinik, the first private clinic owned by a Malay in Malaysia. He was known as a dedicated physician who made house calls at any hour.

Mahathir's political journey began with activism in the 1940s. He joined protests against citizenship policies in the Malayan Union. He eventually joined the United Malays National Organisation, known as UMNO. He entered parliament in 1964 as the Member of Parliament for Kota Setar Selatan. His early career included disagreements with Prime Minister Tunku Abdul Rahman. After these disputes, Mahathir re-entered parliament through the Kubang Pasu constituency. He later served as the Minister of Education and the Minister of Trade and Industry.

During his first term as prime minister, Mahathir drove massive national changes. He promoted industrial privatisation and launched large infrastructure projects. These included the North–South Expressway and the Kuala Lumpur City Centre. His policies helped turn Malaysia into a dynamic emerging economy in Southeast Asia. He also managed the country's recovery from the 1997 Asian financial crisis. He used capital controls and stimulus measures to help the economy. However, his leadership also faced criticism. In 1987, he ordered the detention of activists under Operation Lalang. In 1998, the dismissal of deputy Anwar Ibrahim led to the Reformasi political movement.

Mahathir resigned from office in 2003, but he remained a powerful figure. In 2016, he left UMNO due to the 1MDB corruption scandal. He joined the BERSATU party and led the Pakatan Harapan coalition. This coalition won the 2018 general election, returning him to the prime minister's office. During this second term, he pledged to fight corruption and investigate the 1MDB scandal. He also worked to secure the pardon and release of Anwar Ibrahim. His second tenure ended in 2020 during a political crisis.

Throughout his life, Mahathir's views have been shaped by Malay nationalism and Islamic beliefs.
