After her father lost his job, the family moved to Bombay when she was eight years old. They initially lived in a cowshed in the Malad suburbs. At age seven, she worked at an All India Radio station singing compositions. She soon moved into acting, starting with a minor, uncredited role in the film Basant in 1942. During this early period, she was often credited under the screen name Baby Mumtaz.

As she grew older, Madhubala transitioned from child roles to leading lady status. In 1947, director Kidar Sharma cast her in the drama Neel Kamal. Although the studio initially refused to fund the film, the production was eventually completed. This role allowed her to move away from being a child actor. She soon adopted the professional name Madhubala, which was finalized by director Mohan Sinha. This period marked her rise as a versatile performer capable of handling complex characters.

One of her most significant breakthroughs was the 1949 horror film Mahal.

The director, Kamal Amrohi, chose her for the role to add a sense of mystery to her character. The film became one of the biggest box-office successes of that decade. This success led to a long string of starring roles opposite the most famous leading men of the era. Madhubala also demonstrated her dedication to her craft by learning English, becoming fluent within just three months in 1950.

Madhubala's most iconic performance came in the 1960 historical epic Mughal-e-Azam. She played the character Anarkali in this massive production. The film was the highest-grossing movie in India at the time of its release. Her performance earned her widespread critical acclaim and a nomination for the Filmfare Award for Best Actress. Critics have since described her work in this film as one of the finest in the history of Indian cinema.

Beyond her acting, Madhubala was known for her significant philanthropic efforts. In 1950, she donated 5,000 to help children suffering from poliomyelitis and provided funds for the Jammu and Kashmir relief fund. She also gave 50,000 to assist refugees from East Bengal. While these acts earned her the nickname "queen of charity," they also caused public controversy regarding her religious identity. Consequently, she often chose to conduct her charitable work privately and anonymously.

Despite her professional success, Madhubala faced a lifelong health struggle. She was born with a ventricular septal defect, which is a congenital heart disorder involving a hole in the heart. This condition caused recurring bouts of breathlessness and hemoptysis, which is the coughing up of blood. She kept this diagnosis private for much of her career to protect her work. Ultimately, the condition led to her death in 1969 at the age of 36. She remains a lasting icon of the film industry today.
