Over many centuries, different groups controlled the region. The Ghorids ruled after the Ghaznavids. The city faced destruction by the Mongols in 1220 and the Timurids in 1383. However, Timur later rebuilt the city. By the late 16th century, the Safavid dynasty claimed the area. It fell to the Hotak dynasty in 1709 and was later invaded by Nadir Shah in 1738. By 1747, it became part of the Durrani Empire. In 1880, Mohammad Ayub Khan used the city as a lodging and recruiting place before the Battle of Maiwand.

In the mid-20th century, the city underwent major physical changes. From the 1940s until 1978, Americans worked on the Helmand Valley Authority irrigation project. This project was modeled after the Tennessee Valley Authority in the United States.

American urban planning helped rebuild and expand the city. This included creating broad, tree-lined streets and brick houses. The project used three large canals: the Boghra, the Shamalan, and the Darweshan. These canals turned vast areas of desert into productive farming zones. This allowed many people to move to the area for jobs.

Today, Lashkargah has an estimated population of 191,458 people. The economy relies on agriculture, trade, transport, and tourism. Agriculture is the largest use of land, covering 61% of the area. The city is a center for education with about 27 public schools and universities.
