On May 16, 218, he was declared the emperor. To show he was a true leader, he took the name Marcus Aurelius Antoninus. This was the same name as the former emperor, Caracalla. He won a big battle at Antioch in June 218 to secure his rule.

His family held the hereditary right to serve as high priests for the sun god, Elagabal. This deity was represented by a sacred stone known as a baetyl. The name Elagabal likely comes from the Arabic phrase "Ilāh al-Jabal," meaning "God of the Mountain." Because of his religious background, he became a highly controversial figure in Roman history.

His rise to the imperial throne was driven by his grandmother, Julia Maesa. After the death of the emperor Caracalla, a man named Macrinus took control. To remove Macrinus, Maesa orchestrated a political revolt. She spread rumors that Elagabalus was actually the illegitimate son of Caracalla. This claim helped win the support of the Third Legion Gallica.

Following this victory, Macrinus was executed in Cappadocia. Diadumenian was also executed after being captured at Zeugma. By the middle of June 218, the Senate officially recognized Elagabalus as the emperor.

Once he arrived in Rome in 219, Elagabalus introduced major religious changes. He brought the cult of Elagabal to the capital. He made the worship of his sun god a central part of Roman religious life. He even forced government leaders to participate in these new rites. Elagabalus held many religious titles, including pontifex maximus, the head of the Roman priesthoods. He also identified his god with the Roman sun god, Sol Indiges. This fusion of traditions was a massive departure from established Roman customs.

His reign was marked by significant tension with Roman institutions. Historical accounts from writers like Cassius Dio and Herodian describe his behavior as highly irregular. They claimed he married four women, including a Vestal Virgin, and ignored traditional Roman social norms. These accounts suggest his actions alienated the Senate, the common people, and the Praetorian Guard. Because of this widespread opposition, his rule was very unstable. At the age of 18, Elagabalus was assassinated in March 222. His cousin, Severus Alexander, was chosen to replace him.

Modern historians view the life of Elagabalus with a sense of caution. For centuries, he was remembered as an eccentric and incompetent leader. The historian Edward Gibbon described him as a man who abandoned himself to pleasure.
