In these patients, pressure can build up in the pulmonary arteries. If the pulmonary pressure becomes higher than the aortic pressure, a shunt reversal occurs. This means blood flows from the right side of the heart to the left. 
Diagnosing cyanosis requires a careful physical examination and a detailed medical history. Doctors must account for several factors that can mimic the condition. For example, consuming blue or purple food dyes can change skin color. Large birthmarks, such as Mongolian spots, can also look blue. Certain medications containing silver or amiodarone may cause discoloration. 
Medical professionals use various tests to find the root cause of the blue tint. They may use pulse oximetry to measure oxygen levels in the blood. Other tools include an electrocardiogram (ECG) for the heart or an echocardiogram. For more detailed views, they might use X-rays or CT scans. If a patient has methemoglobinemia, a specific type of hemoglobin disorder, doctors may treat them with methylene blue. 
The history of studying cyanosis shows how much our understanding has grown. The name comes from the Greek word "cyanós," which means blue. It is believed that Jean-Baptiste de Sénac first described it in 1749. He was a French physician who worked for King Louis XV. He discovered the link to heart defects during an autopsy. Much later, in 1919, Dr. Christen Lundsgaard provided a deeper scientific explanation. He calculated the exact concentration of deoxyhemoglobin required to cause the color change.
