Life in Colombia was difficult for many people during this time. While the economy grew, 45 percent of Colombians lived in poverty. Many people lived in slums near big cities. This campaign ended tragically when Galán was assassinated on August 18, 1989. Following this event, Gaviria became the campaign manager and was seen as the political successor. His campaign faced threats from drug trafficker Pablo Escobar. Gaviria narrowly avoided a tragedy when he did not board Avianca Flight 203. The plane exploded, and all 107 people on board were killed. Despite these dangers, Gaviria won the 1990 election with 60% of the vote.

As President, Gaviria managed complex social and security issues. He oversaw the adoption of the Constitution of 1991, which was created by a summoned Constituent Assembly. This new constitution changed how the Colombian government functioned. Gaviria also led efforts against drug cartels and guerrilla factions. His administration built the La Catedral prison for Pablo Escobar. However, Escobar escaped from the prison on July 20, 1992. The drug lord was eventually killed on December 2, 1993. This event was considered a major victory for Gaviria's government.

Economic and social struggles remained a major part of his presidency. Although there was economic growth, 45 percent of Colombians lived below the poverty line. This poverty was especially high in rural areas. Many people also lived in growing slums near major cities. During this time, paramilitary groups known as autodefensas were active. These groups fought guerrilla factions but also committed acts of violence against marginalized people. In 1994, Gaviria created the Convivir network to help the army. Some reports suggested these networks were used by traffickers and landowners for illegal purposes.

After his presidency, Gaviria transitioned to international diplomacy. He served as the Secretary General of the OAS from 1994 to 2004. He was reelected to this position in 1999. One of his major tasks was acting as an international facilitator for the OAS mesa process. This was intended to help resolve the political crisis in Venezuela between 2002 and 2003. Later, he became the leader of the Colombian Liberal Party. He is also a member of the Club of Madrid, an organization of over 100 former democratic leaders.
