Most people live in cities like Birjand, Qaen, or Tabas.

This makes it the third largest province in Iran. It is smaller than Kerman and Sistan and Baluchestan. The province is bordered by several other regions. Razavi Khorasan is to the north. Semnan Province sits to the northwest. Isfahan and Yazd Provinces are to the west. Kerman Province is to the south. Sistan and Baluchestan Province is to the southeast.

The geography of South Khorasan is divided into two distinct regions. The north and northwest are mountainous and elevated. This area contains the tallest mountains in the province. Two of these high peaks are named Kamarak and Shah Kuh. The central and southern areas are different. These are low and flat plains. The climate is generally dry and desert-like. The Parthian empire was based near Merv in Khorasan. During the Sassanid dynasty, the province had a specific government. A Spahbod, or Lieutenant General, governed the area. There were also four margraves to command different parts.

Many empires and dynasties fought for control of this land. In 651, Islamic Arab armies invaded Khorasan. The Abbasid clan held the territory until 820. Later, the Taherid clan ruled in 896. The Samanid dynasty arrived in 900. In 994, Mahmud of Ghazni conquered the region. The Seljuq empire also gained control of Nishapur in 1037. Later, the Khwarazmids conquered the area in 1157. The Mongols also annexed the territory into the Mongol Ilkhanate. In the 14th century, the Sarbedaran movement sought independence. Tamerlane took control in 1468 and made Herat a capital. Later, Uzbek tribes occupied the area in 1507.

Modern South Khorasan was created through administrative changes. It was one of three provinces made from the old Khorasan. This division happened on September 29, 2004. Before this, Khorasan was the largest province in Iran. The new province originally only included Birjand and a few counties. These were Nehbandan, Darmian, and Sarbisheh. Over time, more areas were added. The northern and western parts of old Quhistan were annexed. This included cities like Qaen, Ferdows, and Tabas. Today, the province is a mix of new and old territories. It is built upon the historical land of Quhistan.

The province is famous for several unique things. The people are mostly Persian. South Khorasan is well known for its famous rugs. It is also a major producer of saffron.
