His family also had connections to the Inca emperor Huayna Capac. During his childhood, his family lived in Belgium and New York City. His father served as the Chilean ambassador to the United Nations. Piñera eventually returned to Chile to attend the Colegio del Verbo Divino. He graduated from the Pontifical Catholic University of Chile in 1971. Later, he earned a Master of Arts and a PhD in economics from Harvard University.

Before entering politics, Piñera built a massive personal fortune. He was a very successful businessman in several different industries. He helped introduce credit cards to Chile during the late 1970s. He also held major interests in LAN Airlines and the television channel Chilevisión.

Forbes estimated his net worth at US$2.7 billion at the time of his death. This made him the third richest person in Chile. However, his business career included several legal challenges. In 1982, he faced an arrest warrant regarding banking laws at Banco de Talca. He was eventually acquitted by the Supreme Court. He also faced fines from the SVS, Chile's securities regulator, regarding stock information.

Piñera's political journey began in the Senate. He represented the East Santiago district from 1990 to 1998. He first ran for the presidency in the 2005 election. He lost that election to Michelle Bachelet. He ran again in the 2009–2010 election cycle. In the first round, he received 44.05% of the votes. He then entered a run-off election against Eduardo Frei Ruiz-Tagle.

Piñera won the run-off with 51.61% of the total votes. To govern fairly, he suspended his membership in the National Renewal party. This allowed him to act as an independent leader for the country.

His first presidency was marked by major national crises. In 2010, a massive earthquake struck Chile. Piñera led the efforts to rebuild the nation after the disaster.

That same year, 33 miners became trapped deep underground. Piñera was a visible leader during their successful rescue. His second term, starting in 2018, involved different challenges. He managed the country's response to the COVID-19 pandemic. During this time, he also oversaw the legalization of same-sex marriage in Chile.

Piñera's leadership also faced intense social unrest. His administrations dealt with two very large protest movements. The first was the student protests in 2011. The second was a much larger and more violent movement in 2019 and 2020. These protests were the largest since democracy returned to Chile in 1990. Despite these tensions, he maintained a relationship with his successor. He developed an amicable relationship with President Gabriel Boric. This was notable because Boric had previously been a harsh critic of Piñera.

Piñera's influence extended into social and environmental work. He created the Enterprising Women Foundation to help young women of lower income. He also started the Fundación Futuro to support democracy and justice.

For environmental protection, he created Tantauco Park on Chiloé Island. This is a private natural reserve that is open to the public. He passed away on February 6, 2024. He died at age 74 following a helicopter crash on Lake Ranco. His followers, known as "Piñerism," continue to represent a center-right faction in Chile.
