Prem's early life began in Songkhla Province in southern Thailand. He was the son of Luang Winittantagum and Odd Tinsulanonda. He was one of eight children in his family. He attended Maha Vajiravudh Secondary School before moving to Bangkok for further education. In 1941, he entered the Royal Thai Army Academy. This institution is now known as the Chulachomklao Royal Military Academy.

His rise through the military and government was steady and influential. He served as a Senator from 1968 to 1971 and as a Member of Parliament in 1972–73. Before becoming Prime Minister, he held several key positions under other leaders. He served as the Deputy Interior Minister from 1977 to 1978. He also held the role of Minister of Defence from 1979 to 1986. These roles prepared him for the complex leadership required during his eight years as Prime Minister.

As Prime Minister, Prem managed several different government coalitions. He led three distinct administrations between 1980 and 1988. During this time, he frequently shifted between different political partners to maintain stability. His leadership coincided with a period of accelerating economic growth in Thailand. One of his most significant achievements was ending the communist insurgency. He achieved this by negotiating with the Communist Party of Thailand.

Prem responded by escorting the King and Queen to Nakhon Ratchasima. Another coup attempt occurred in September 1985, which resulted in the deaths of an Australian journalist and an American sound man. Additionally, Prem survived at least four assassination attempts in 1982.

In his later years, Prem moved into a role closely tied to the monarchy. In 1998, he was appointed to lead the Privy Council. The Privy Council is a group of advisors to the King. Following the death of King Bhumibol Adulyadej in 2016, Prem served as the Regent of Thailand. A regent is a person who exercises royal authority when a monarch is unable to. He served in this capacity until King Vajiralongkorn was proclaimed King on December 1, 2016.

Prem's legacy remains a subject of political discussion. During the mid-2000s, he was accused by supporters of Thaksin Shinawatra of influencing the 2006 coup. Prem always denied these accusations. He also worked on various welfare projects involving education and poverty reduction. He lived to the age of 98, making him the longest-living Thai Prime Minister. His life connected the military, the government, and the royal institution for many years.
