The disease develops through two primary mechanisms. Primary pellagra occurs when a person's diet lacks sufficient niacin or the amino acid tryptophan. The body can actually convert tryptophan into niacin, so a lack of both leads to a deficiency. Secondary pellagra occurs when a person consumes enough niacin, but their body cannot use it properly. This might happen because of chronic alcoholism or certain medical conditions like Hartnup disease, a genetic disorder that reduces tryptophan absorption.

Many different factors can trigger secondary pellagra by interfering with nutrient absorption or metabolism. For example, carcinoid syndrome involves neuroendocrine tumors that use tryptophan to produce serotonin. In healthy patients, only 1% of dietary tryptophan becomes serotonin, but in these patients, that number can rise to 70%. This leaves very little tryptophan available for the body to make niacin. Additionally, certain medications can cause problems. The antibiotic isoniazid binds to vitamin B6, which is a required cofactor for the reaction that turns tryptophan into niacin. Other drugs, such as chloramphenicol and fluorouracil, can also provoke the condition.

Symptoms of pellagra affect many different systems in the body. Dermatitis is a major sign, often appearing as a "Casal collar," which is a characteristic rash around the neck.

This skin inflammation typically occurs in areas exposed to friction or radiation. Other physical signs include hair loss, weakness, and a smooth, beefy red glossitis, which is inflammation of the tongue. The disease also impacts the nervous system, causing ataxia, which is a lack of coordination, or even paralysis. Mental symptoms can include confusion, aggression, or psychomotor disturbances like restlessness and tension.

Historically, scientists struggled to understand the cause of pellagra. In 1735, Gaspar Casal first described the skin effects in Spain, leading to the term "Asturian leprosy." By the 19th century, researchers in Italy and France debated whether the disease was a form of scurvy or caused by a toxin. Some even believed it was carried by insects, similar to malaria. In the early 1900s, pellagra became an epidemic in the Southern United States. Between 1906 and 1940, more than 3 million Americans were affected, and over 100,000 people died from the disease.

The relationship between food preparation and health is a key part of the pellagra story. Pellagra is common in areas where corn, or maize, is a staple food. This involves treating corn with an alkali solution, such as lime, to make the niacin nutritionally available. When maize is eaten globally without this process, pellagra becomes more common. Today, the disease is often seen in the developing world, particularly in sub-Saharan Africa, or among displaced populations and refugees who rely on limited food aid.
