His successor, Muhsinzade Mehmed Pasha, also preferred peace. Mustafa III, however, insisted on war with Russia. This disagreement led to the resignation of Mehmed Pasha in 1768. The Sultan believed the Ottomans would achieve an easy victory. In reality, the empire was unprepared for a prolonged conflict. The war resulted in major territorial losses. Russian armies occupied parts of Crimea, Romania, and Bulgaria.

Despite the difficulties of war, Mustafa III continued to invest in infrastructure and education. He attempted military reforms with the help of a French officer named François Baron de Tott. These reforms included modernizing the artillery corps. In 1773, he also founded the Naval Engineering School. Mustafa III was also a prolific builder. He rebuilt the Fatih Mosque, which had been constructed by Mehmed the Conqueror. He also built the Laleli Mosque complex. He led many construction projects following the earthquakes of 1766 and 1767.

Beyond politics, Mustafa III had a rich personal life and intellectual interests. He was a poet who wrote under the pseudonym Cihangir. His poetry often reflected on the nature of the world and fate. He had several consorts, including Mihrişah Kadın and Aynülhayat Kadın. He had many children, including his son Selim III, who would later become sultan. Mustafa III died of a heart attack on January 21, 1774, at the Topkapı Palace. He was buried in his own mausoleum at the Laleli Mosque. His death left the empire facing difficult economic and administrative challenges. His brother, Abdul Hamid I, succeeded him as sultan.
