Investigators found he had interests in Venezuela and links to drug traffickers in Colombia. His networks also reached into Belgium and Germany. He maintained strong ties to Mafia families in Palermo through his relatives.

His long period as a fugitive finally ended in 2023. On January 16, 2023, police arrested him near a private clinic in Palermo. He was using a false name because he was undergoing chemotherapy. He had been diagnosed with colon cancer. Denaro died in a prison hospital on September 25, 2023. He fell into an irreversible coma before passing away at age 61.
