Melgarejo had a very fast rise in the military. He joined the army in 1836 when he was only 16. He started as a simple private. He fought in the War of the Confederation. In 1841, he fought in the Battle of Ingavi.

After his release, Melgarejo continued to influence politics. He helped overthrow President Jorge Córdova in 1857. Later, he helped President José María de Achá take power in 1862.

His rise to the presidency was tied to personal and political chaos. In 1864, rumors spread regarding an affair between Melgarejo and President Achá's wife. This scandal caused the president to lose respect within the army. Melgarejo took advantage of this instability. He launched a coup against Achá in December 1864. Once in power, he faced many challenges. A group called the Constitutionalists led rebellions against him between 1865 and 1866. He also faced a civil war when former President Belzu returned from exile. On March 27, 1865, Melgarejo's forces killed Belzu in La Paz.

Melgarejo's presidency had lasting negative effects on Bolivia. He signed unfavorable border treaties with Chile in 1866. He also signed a treaty with Brazil in 1867. These agreements caused significant problems for the country in later years. His administration was also noted for its treatment of indigenous populations. His leadership style was often described as despotic. This means he ruled with absolute power and often ignored the law.

His rule ended through a violent uprising in 1871. General Agustín Morales led the army against him. The people of La Paz also supported the revolt. A very fierce battle occurred within the city of La Paz. The uprising successfully deposed Melgarejo on January 15, 1871. Melgarejo fled to Chile to escape the conflict. He later traveled to Lima, Peru, to find his lover, Juana Sánchez. On November 23, 1871, he was shot and killed in Lima. The act was carried out by Juana's brother, José Aurelio Sánchez.
