In 1886, the Egyptian garrison finally surrendered to the Mahdists. Italy took control of the town in 1894. They built strong defenses to keep the area stable. By 1896, Anglo-Egyptian forces moved back in to take control.

Kassala has a complex history shaped by many different powers. In 1834, the Egyptian Army established the city as a garrison town. This happened after the Egyptian conquest of Sudan in 1821. Later, the Mahdist state was formed, and the Egyptian garrison was besieged. In 1884, Abyssinian forces led by Ras Alula tried to help the Egyptians. They won the Battle of Kufit, but they could not break the defenses. The Egyptian garrison eventually surrendered to the Mahdists in 1886.

Control of the city changed hands again in the late 19th century. Italy annexed Kassala to Italian Eritrea on July 17, 1894. The Italians fortified the town and defeated the Mahdists at the Battle of Kassala. However, Italy lost the Battle of Adwa in 1896. This defeat forced them to evacuate the town. Anglo-Egyptian forces then re-occupied the area and joined it to Sudan. In the early 1900s, a railway system was built to connect the city. It first linked Kassala to Port Sudan in 1906. By 1929, the tracks extended to Sennar in the south.

The city's economy grew through agriculture and transport. Horticulture, which is the science of garden cultivation, began near the Gash and Barka rivers. Later, the British introduced cotton cultivation to the region. The Kassala Cotton Company helped manage these agricultural developments. During the East African Campaign in 1940, the city saw military conflict. Italian forces attacked with 2,500 men and 24 tanks. They defeated a British garrison of 1,300 colonial soldiers. The Italians occupied the city until mid-January 1941, when British forces returned.

Kassala is a very diverse place with many different ethnic groups. These include the Beja, Beni-Amer, Amarar, Bishari, Rashaida, and Halanga. There are also many immigrants from northern and western Sudan, such as the Fula and Nuba. In 2003, the population was recorded at 530,950 people. The city has also hosted many refugees due to regional conflicts. During the Ethiopian-Eritrean War from 1998 to 2000, about 110,000 Eritreans fled to the area. More recently, the city faced drone attacks during the Sudanese civil war in May 2025. These attacks targeted the fuel storage at the Kassala Airport.

Cultural and educational institutions are central to the city. The Khatmiyya Mosque is the most important cultural site in Kassala.
