Neonatal mortality refers to deaths occurring within the first 28 days after birth. In developing countries, neonatal death accounts for 40% to 60% of all infant mortality. Postneonatal mortality includes deaths of children aged 29 days to one year. Major contributors to postneonatal death include malnutrition, infectious diseases, and problems in the home environment.

One major cause of infant mortality is preterm birth, or birth before 37 weeks of gestation. A full-term birth occurs at 40 weeks. Preterm births are subdivided into mild (32–36 weeks), very preterm (28–31 weeks), and extremely preterm (under 28 weeks). These births can be spontaneous or medically induced. Spontaneous births happen due to natural labor or the rupture of the amniotic sac. Medically induced births occur when continuing a pregnancy poses risks to the parent or fetus. These risks include conditions like preeclampsia or diabetes.

Preterm infants face high risks because their organ systems are not fully developed. The respiratory system is often affected, leading to conditions like respiratory distress syndrome or apnea. Other systems, such as the gastrointestinal, immune, and cardiovascular systems, also require more time to mature. Many preterm births are associated with inflammation in the body. Scientists look for biomarkers of inflammation, such as C-reactive protein or cytokines, to understand these risks. In low-income countries, the survival rate for infants born before 28 weeks is only 10%. In high-income countries, that survival rate is 90%.

Another significant cause is Sudden Infant Death Syndrome (SIDS). SIDS is the sudden death of an infant under one year of age where no cause is detected after an investigation. It is more common in Western countries. Researchers use a "triple-risk model" to study SIDS. This model suggests three factors contribute: smoking during pregnancy, the age of the infant, and certain sleeping conditions. These conditions include sleeping on the stomach, overheating, or covering the face. The "Back to Sleep" campaign encouraged sleeping on the back, which lowered SIDS rates by 50%.
