The geography of Ilam is divided into two distinct natural areas. The northern and eastern parts are mountainous. These mountains run in a northwest-to-southeast direction. They are separated by plains and rolling hills. These hills are often used for orchards and crops. The most prominent range is the Kabir Kouh.

This range stretches 160 km from Pol-e Zal to Mishkhas. The highest point is Kan Seifi peak. It reaches an elevation of 2,775 meters. Other notable peaks include Manesht and Chaman Ghir.

The southwestern part of the province is very different. It consists of low-lying plains. These plains have elevations between 0 and 200 meters. Despite low precipitation, these areas are heavily cultivated. This is possible through irrigation networks and dams. Rivers flow from the eastern mountains to these lands. The Ghamasiab river flows into Ilam and becomes the Seymareh river.

Ilam has a deep history tied to ancient civilizations. The name "Ilam" comes from Elam. Elam was a prehistoric civilization. It ruled southwestern Iran from 2700 BC to 539 BC. Human settlement in this area dates back to 5000 BC. The Assyrian king Ashurbanipal destroyed Elam in 639 BC. Later, Elam was part of the Median and Achaemenid Empires.

During the Sasanian era, the area had two states. These were Mehrjanqazaq in the east and Masbasan in the west. Following the Muslim conquest in 640 AD, Arabs called the area "Jibal." This name means "Land of the Mountains."

Many different groups have ruled the region over the centuries. The Buyid Dynasty controlled the area until 1055. From 1155 to 1424, Kurds ruled the Zagros Mountains. The Hazaraspids were composed of Little Lor and Big Lor groups.

In 1908, Gholamreza Khan Feyli built the Vali Castle. It now serves as the Ilam Museum of Anthropology.

The 20th century brought significant political changes to the province. The Pahlavi dynasty began in 1925. In 1928, government forces took over Poshtkouh. This ended the 332-year rule of the Feyli family. In 1929, the name "Hossein-Abad" was changed to "Ilam." The province's status changed several times through various laws. In 1964, several districts were annexed to Ilam. It finally became a full province in March 1974. However, the Iran-Iraq war devastated the province's infrastructure. The war began with an Iraqi invasion in 1980.

Ilam is home to rich biodiversity and unique wildlife. About 31% of the territory is covered by forests.
