Human history in this region extends back tens of thousands of years. Archaeological sites like Howieson's Poort provide evidence of very early human behavior. These sites date back between 62,000 and 65,000 years. Researchers found extremely old evidence of bow-and-arrow hunting there. They also discovered shell-bead jewellery from that era. Other sites, like Amanzi Springs, contain Earlier Stone Age artefacts. These items were found alongside well-preserved plant and animal remains.

Hunters and gatherers ancestral to the San lived here at least 10,000 years ago. Later, agriculturalist populations ancestral to the Xhosa people assimilated into the area.

European contact began with Portuguese explorers in the late 15th century. Bartholomeu Dias landed on St Croix Island in 1488. Vasco da Gama noted Bird Island in 1497. For a long time, European charts only called the area a landing place with fresh water. In 1799, British troops built Fort Frederick to deter French landings.

The modern city was founded in 1820 by Sir Rufane Shaw Donkin. He was the Acting Governor of the Cape Colony. He named the settlement Port Elizabeth after his wife, Elizabeth Frances. This was done to honor her after her death in India.

During the apartheid era, the city experienced significant social and political struggle. The government used the Group Areas Act to forcibly relocate non-white populations. Between 1965 and 1975, residents of South End and Fairview were moved. This happened because those areas were considered prime real estate. The city became a center for civil rights activism. In 1952, the Defiance Campaign saw 20,000 people participate in Gqeberha. Raymond Mhlaba and Florence Matomela were among the first to be arrested. Later, in 1985, women in the townships suggested a consumer boycott to weaken apartheid institutions. This economic boycott received massive support in the townships.

In recent years, the city has undergone an official name change. In 2019, the Eastern Cape Geographical Names Committee recommended the name Gqeberha. This name is a Xhosa word for the Baakens River. This river flows through the heart of the city. The name change was officially gazetted on 23 February 2021.
