The museum's physical structure is quite large and organized. The current building was completed in 1998 in the Kulturforum museum district. Architects Heinz Hilmer and Christoph Sattler designed this modern space. The building contains 72 rooms and provides a floor area of two kilometers. A large central hall acts as a "meditation hall" for visitors. This hall is mostly empty to allow for easy movement between rooms. It also provides a space where school groups can sit.

The history of the collection began with purposeful government acquisitions. Unlike many royal collections, this museum was built through purchases by the Prussian government starting in 1815. One major purchase included 160 paintings from the collection of Marchese Vincenzo Giustiniani. In 1821, the museum acquired 677 paintings from the English merchant Edward Solly. Solly was a collector of Italian Trecento, Quattrocento, and Early Netherlandish paintings. Later, the director Gustav Friedrich Waagen acquired 345 more works between 1830 and 1868.

As Berlin became the capital of the German Empire in 1871, the collection grew rapidly. The art historian Wilhelm von Bode became a key figure during this era. He led the sculpture and painting collections and helped the museum reach international prominence. The collection faced great challenges during World War II. While many works were moved to shelters, 400 large paintings were destroyed in a fire in a Flak tower. Additionally, several hundred paintings were lost due to looting by soldiers.
