International pressure, particularly from Nigeria, forced Gnassingbé to hold new elections. He resigned from his interim position in February 2005. He then ran in a presidential election on April 24, 2005. Official results stated he won with slightly more than 60% of the vote. However, the European Union and the Carter Center called the election fraudulent. The election was also marked by violence. Protests by opposition groups led to the deaths of over 1,000 citizens by security forces. Additionally, 40,000 refugees fled to neighboring Benin and Ghana.
Gnassingbé continued to lead Togo through several subsequent election cycles. He was re-elected in 2010, 2015, and 2020. In the 2015 election, he won with approximately 59% of the vote. In 2020, he won a fourth term with about 72% of the vote. During his long presidency, he focused on national infrastructure projects. He helped mobilize 12 billion 860 million CFA francs for construction. He also oversaw the completion of the Togblécopé and Amakpapé bridges. Some critics, however, have accused his administration of corruption regarding the management of the phosphate sector.
In recent years, the structure of the Togolese government has undergone a massive transformation. In 2024, the parliament approved amendments to the constitution. These changes moved Togo from a presidential republic to a parliamentary system. This new system is known as the Fifth Republic of Togo. The amendments stripped most powers from the presidency. These powers were transferred to a new role called the President of the Council of Ministers. 
The new government structure officially took effect on May 3, 2025. Gnassingbé was sworn in as the inaugural President of the Council of Ministers. This role is the most powerful position in the country. Meanwhile, Jean-Lucien Savi de Tové became the first ceremonial president. Opposition members have criticized these reforms. They argue that the changes allow Gnassingbé to remain in power indefinitely. This is because the new position of President of the Council of Ministers has no term limits. Since June 2025, protests have grown in Togo demanding his resignation.
