In 753 AD, a document mentioned Ferrara as part of the Exarchate of Ravenna. Later, a leader named Tedald of Canossa held the land. After the death of Matilda of Canossa in 1115, the city became a free municipality. This free city lasted for about 150 years. Finally, Azzo VI d'Este began the family's long lordship in 1208.

The House of Este began its lordship in 1208 with Azzo VI d'Este. This family also ruled the lands of Modena and Reggio. Their political status was quite complex. Ferrara was formally part of the State of the Church. This meant the Este lords were feudal lords for the Pope. However, Modena and Reggio belonged to the Holy Roman Empire. Therefore, the family served the Emperor for those territories. In 1471, Borso d'Este became the Duke of Ferrara. Pope Paul II granted him this title.

During the late 15th and early 16th centuries, Ferrara became a world-class cultural center. Ercole I d'Este was a major patron of the arts. They included singers named Laura Peverara, Anna Guarini, and Livia d'Arco. These performers were considered true virtuosos.

Natural and political changes eventually altered the Duchy. A major earthquake hit the city in 1570. In 1597, Alfonso II died without a legitimate male heir. His cousin, Cesare d'Este, attempted to inherit the lands. However, Pope Clement VIII did not recognize this succession. The Pope claimed Ferrara as a vacant fief. This brought Ferrara under direct papal rule. The House of Este kept Modena and Reggio until 1796.
