Guinea worm disease is a sickness caused by a tiny worm.

People get sick by drinking water with tiny water creatures in it. These creatures are called copepods. When a person drinks the water, stomach acid breaks the copepods. This lets out the tiny worm larvae. The larvae move into the body and grow. A year later, a large female worm moves toward the skin. It usually forms a painful blister on a leg or foot.

Dracunculiasis is a sickness caused by a tiny roundworm called *Dracunculus medinensis*. This worm is also known as the Guinea worm. It is a very hard disease for people to live with. It can make it very difficult for people to walk or work. In some places, people even call it the "empty granary disease." This is because sick farmers cannot work in their fields to grow food.

The way this sickness works starts with a tiny water creature. These small creatures are called copepods. When a person drinks water that has copepods in it, the infection begins. The acid in the stomach digests the copepod. This releases the tiny Guinea worm larvae inside the body. The larvae move into the stomach and intestines to hide. They grow into adult worms over several months.

About one year after the infection, the female worm moves toward the skin. It usually travels to a lower leg or a foot. The worm creates a very painful, fluid-filled blister. When the blister bursts, the worm slowly comes out of the body. This can take anywhere from three to ten weeks. There is no medicine to kill the worm or stop the sickness. Doctors must wrap the worm around a small stick or gauze. They turn the stick each day to pull the worm out slowly.

History shows us how much this disease used to hurt people. In the 1900s, up to 48 million people were sick every year. This happened mostly in parts of Africa and South Asia. In 1986, the World Health Assembly started a plan to stop the disease. By 1995, almost every country had a program to fight it. The number of cases has dropped very fast since then. In 1997, there were fewer than 100,000 cases each year. By 2025, there were only 10 cases reported.

We can prevent this sickness by using clean water. People can use nylon filters or special filter straws to drink safely. These tools catch the tiny copepods before they are swallowed. It is also important not to soak painful wounds in drinking water. This stops the worm from releasing more larvae into the water. If we succeed, this could be the second human disease ever removed from the world. This would happen after the disease called smallpox was gone.

Dracunculiasis is a parasitic infection caused by the roundworm *Dracunculus medinensis*. It is commonly known as Guinea-worm disease. This disease is highly debilitating for those it affects. It often causes significant physical disability in about half of all infected people. Because it can prevent farmers from working during harvest seasons, some communities call it the "empty granary disease."

The infection process begins with tiny aquatic crustaceans called copepods. These small creatures carry the *D. medinensis* larvae. Humans typically become infected by drinking water contaminated with these copepods. Sometimes, humans eat fish or frogs that have already consumed the infected copepods. Once inside the human body, stomach acid digests the copepod. This process releases the larvae, which then penetrate the digestive tract. The larvae move into the abdomen or the retroperitoneal space, which is the area behind the organs near the back.

Inside the body, the larvae undergo several developmental stages. Over the next two to three months, they grow into adult male and female worms. The male worm is smaller in both length and width. The female worm is much larger. After the worms mate, the male dies. The pregnant female then migrates through connective tissue or along bones. She continues to develop her embryos during this migration. About one year after the initial infection, the female reaches the skin, usually on a lower leg or foot.

As the female worm prepares to emerge, she creates a painful, fluid-filled blister. This blister can cause allergic reactions like fever, dizziness, or nausea. Within one to three days, the blister grows and bursts into an open wound. The wound is intensely painful as the worm slowly exits the body. This emergence can take between three and ten weeks. If the person submerges the wound in fresh water to relieve the pain, the female releases hundreds of thousands of larvae. This milky-white substance continues the life cycle by infecting more copepods in the water.

There is currently no medication to kill the worm or prevent the disease once infection occurs. Treatment focuses on the careful physical removal of the worm. Medical providers wrap the emerging worm around a small stick or piece of gauze. This maintains gentle tension to encourage the worm to exit. The stick is turned each day as a few more centimeters emerge. It is vital not to apply too much tension. If the worm breaks inside the wound, it can cause severe swelling and pain.

History shows how much the world has changed regarding this disease. In the early 20th century, dracunculiasis was widespread in Africa and South Asia. It affected as many as 48 million people every year. In 1986, the World Health Assembly began a global eradication effort. Since then, the number of cases has dropped precipitously. In 1997, cases fell below 100,000 per year. By 2012, there were fewer than 1,000 cases. By 2025, the number had fallen to only 10 reported cases.

Prevention is the most effective way to stop the spread. Since there is no vaccine, efforts focus on cleaning water supplies. People can use nylon filters, fine cloth, or specialized filter straws to remove copepods. Water can also be treated with a larvicide called temephos or by boiling it. Public education is also important to stop people from soaking wounds in drinking water. If these efforts continue, the World Health Organization hopes to achieve total eradication by 2027. This would make Guinea-worm the second human disease ever eradicated, following smallpox.
