They noted high levels during the 2007 Calgary World Cup and the 2009 Hamar World Championships. Consequently, the ISU banned her from all competitions for two years.

Pechstein fought the ban through many legal channels. She argued that her reticulocyte percentage of 3.49 was within a normal range for women her age. She claimed the ISU threshold of 2.4 was too low. Pechstein also suggested her blood levels were affected by an inherited medical condition. She cited her hemoglobin and hematocrit levels as evidence to support her case. In 2010, a medical expert suggested she might have spherocytosis. This is a light form of blood anemia that can be inherited from a parent. However, the Court of Arbitration for Sport (CAS) upheld the ban in 2009. They found no evidence of an inherited condition in the expert testimony.

The legal dispute lasted for many years. Pechstein appealed to the Swiss Federal Supreme Court and the European Court of Human Rights. In 2016, the Federal Court of Justice of Germany recognized the jurisdiction of the CAS. Later, in 2022, the higher Federal Constitutional Court of Germany ruled that her fundamental rights had been violated. This ruling sent her claims for damages back to the lower courts for a re-trial. Despite these struggles, Pechstein returned to competition in 2011. She won a bronze medal at the 2011 World Championships in the 5000 meters.

Pechstein’s longevity in sports is truly remarkable. She participated in eight different Winter Olympic Games. This achievement set a new record for the most Winter Olympics attended by a female athlete. In 2022, she represented Germany as the flag-bearer during the opening ceremony in Beijing. Her career even inspired a shift in interests. In 2018, she expressed interest in becoming a competitive cyclist. She planned to enter the German Track Championships in Berlin. She believed her years of skating training would help her succeed on a bicycle.
