Roy's literary career began with screenplays and film work. She acted in the 1985 film Massey Sahib. She also wrote screenplays for films like In Which Annie Gives It Those Ones and Electric Moon. The film In Which Annie Gives It Those Ones was based on her time as an architecture student. She won a National Film Award for Best Screenplay in 1988 for this work. Before her novel, she studied architecture at the School of Planning and Architecture in New Delhi. She later held a position with the National Institute of Urban Affairs.

Her early life provided much of the foundation for her storytelling. She was born on November 24, 1961, in Shillong. At that time, Shillong was part of the undivided state of Assam. Roy grew up in a Christian family. Her mother, Mary Roy, was a Malayali Christian women's rights activist. Her father, Rajib Roy, was a Bengali Christian tea plantation manager. After her parents divorced when she was two, she moved to Kerala with her mother and brother. She attended schools in Kottayam, Kerala, and Lovedale, Tamil Nadu.

The success of The God of Small Things was massive and immediate. The novel was a major commercial success from its release in May 1997. By the end of June that year, it had been sold in 18 different countries. It reached the fourth position on The New York Times Bestsellers list for Independent Fiction. While many American critics called it "extraordinary" or "dazzling," some UK critics were less favorable. In India, she faced charges of obscenity regarding the book's descriptions of sexuality. Despite this, the book remains a landmark in contemporary literature.

Roy has spent much of her later career writing nonfiction and engaging in activism. She is a spokesperson for the anti-globalization movement. She frequently criticizes neo-imperialism and certain foreign policies. Her writing includes many essays on contemporary politics and culture. These were collected in a five-volume set by Penguin India in 2014. In 2019, her nonfiction was released in a single volume titled My Seditious Heart.

Her activism often focuses on environmental and human rights issues. She campaigned strongly against the Narmada dam project. She argued the dam would displace 500,000 people without proper compensation. Roy donated her Booker Prize money to the Narmada Bachao Andolan to support this cause. This activism led to legal challenges, including a symbolic one-day imprisonment for contempt of court. She has also been a vocal supporter of Kashmiri independence. This stance led to her being charged with sedition by the Delhi Police in 2010.

Roy views the relationship between art and politics as deeply connected. She believes that artists require a certain amount of freedom to function. She has stated that people need space to breathe and create in order to make art. Her writing is influenced by a wide range of figures. These include William Shakespeare, Toni Morrison, and Maya Angelou. She believes that everyone, including oppressors and revolutionaries, has something to teach a writer. In 2024, she won the PEN Pinter Prize. She chose to share this award with the imprisoned writer Alaa Abd El-Fattah, naming him the "Writer of Courage."
