Following the death of Ruhollah Khomeini in 1989, a new leadership process began. The Assembly of Experts met to choose a new supreme leader. Khamenei initially had doubts about his own religious qualifications. At the time, the constitution required the leader to be a marja'. A marja' is a high-ranking religious authority. To allow his leadership, the Assembly of Experts amended the constitution to remove this requirement. This change allowed Khamenei to be confirmed as the supreme leader later that year.

Khamenei's leadership focused on making Iran a powerful energy state. He supported economic privatization for state-owned industries. Because of Iran's large oil and gas reserves, he helped the nation become an "energy superpower." He also focused heavily on the Islamic Revolutionary Guard Corps, or IRGC. He transformed the IRGC into a primary tool for regional influence and domestic control. His foreign policy was centered on Shia Islamism and spreading the ideals of the Islamic Revolution.

Under his rule, Iran became involved in many regional conflicts. He supported the "Axis of Resistance" coalition in several wars. These included the Syrian civil war, the war in Iraq, the Yemeni civil war, and the Gaza war. He also provided support to Russia during the Russo-Ukrainian war.
